I'm helping a user with their project which is a module. It's a best practice to close up the pads which connect the layout to the outside world using a negative value on Solder Mask Expansion. After doing this, 2D looks good, as does an online Gerber viewer, but 3D shows the pad as still having a solder mask opening.
